Our Quality Coordinators Make Care Personal
Our Quality Coordinators play an important role in helping members stay healthy and informed. During their home visits, they provide screenings and checkups based on what each member needs. These may include:
- Checking blood pressure or blood sugar levels
- Providing and explaining simple screening kits
- Talking with you about what your results mean and what comes next
- Making sure your results reach your provider so your care team stays connected
They work closely with our broader care team to make preventive care simple and convenient, helping you take charge of your health with confidence.
